Since August 2012, general practitioners (GPs) and obstetricians and gynaecologists (O&Gs) have been able to prescribe medical abortion drugs to patients through their practice, after the medication was listed by the Therapeutic Goods Administration (TGA). The medication, marketed in Australia as MS 2-Step, is also listed on the Pharmaceutical Benefits Scheme, and is available for use to end pregnancies less than nine weeks gestation.
By providing medical termination of pregnancy (MTOP), GPs can now provide effective help to women when they are faced with an unplanned pregnancy. The inclusion of MTOP to the mantle of care a GP can provide will extend the holistic service they can offer to women through their practice.
MS 2-Step comprises two medications: mifepristone is a medication that blocks the effects of progesterone, which is needed to sustain a pregnancy; and misoprostol, which causes cervical softening and uterine contraction to evacuate the products of conception from the uterus. When used together in early pregnancy (up to 63 days LMP) they are a safe and effective pregnancy termination method.

In order to prescribe and use the medication appropriately our patients require three visits, preferably in attendance with their partner or a support person. All of these three visits are generally longer consultations. We can also arrange Anti D immunoglobulin injections for our patients with negative blood types, arranging anti D products through the Red Cross Blood Bank Service.

During the first visit we overview the general health of the patient and discuss potential alternative options to termination/abortion in order to provide informed consent.
We also talk about abortion options: both surgical and medical. In doing so we introduce women to potential benefits and risks of medical termination, it side effects and also the necessary tests in order to prescribe MS 2-Step.
Our patient will receive a pathology referral for routine blood tests to assess her general health, blood group and antibody; urine test for detecting sexually transmitted diseases; and a referral to have a dating ultrasound – which determines if the pregnancy is in uterus and the age of the pregnancy in day (MS 2-Step is only approved for pregnancies up to 63 days).
It is important to talk about contraception in order to decide which one to use straight after the procedure.
A follow up visit is usually scheduled for a few days following the above mentioned tests, it is during this consultation that we again talk about potential side effects of medications and surgical abortion as an alternative.
Providing the patient is comfortable with proceeding with the medical termination – the doctor would prescribe MS 2-Step and be given guidance on how to take the medication. On the same day the patient will be given a referral for another blood test to be completed in 3 weeks to confirm that the pregnancy had ended. It is our goal to prescribe appropriate contraception on the day with instructions how and when to start.
During the third visit we confirm the success of the procedure by reviewing blood test results, assess general health and counsel on future pregnancies and contraception.

What’s involved with a Pre-Employment Medical?
The candidate/employee will be provided with a questionnaire, which they will go through in detail with our nurse and doctor. Once this is completed any further identified or required testing will be performed. Certified reporting with all the credentials of our clinic and the consulting practitioner will be provided to the employee or management in a timely manner.
All records will be kept on file securely and always adhering to minimum privacy and security recommendations adhered to in Australia. What to expect from your medical assessment?
Depending on the individual and the role description, we will tailor an assessment for your employee. A questionnaire will be sent to you and this will be gone through in detail with our Practice Nurse and our Doctor to ensure a set of further test is flagged. They will then undergo on the spot and off-site testing within 2-3 hours of their arrival.
Following up once tests and relevant summaries have arrived to our clinic, we will identify areas which may need addressing or treatment and convey these to your employee with the performance of any follow up testing or examination if something does arise.
A third session is left as a possibility if there is any further monitoring that has been ordered as a result of the second session. A full report is written up for all interested parties to keep. On request a copy is also sent to the employees General Practitioner to have on file. Our doctors are made available to convey anything queried by their doctor as a measure of courtesy and continuity.
